Patella Stabilisation and MPFL Reconstruction

Overview

Patella instability occurs when the kneecap slips partly or completely out of its groove. This can cause pain, swelling, apprehension, recurrent dislocation and difficulty trusting the knee during sport or stairs.

The medial patellofemoral ligament, or MPFL, is a key soft tissue restraint that helps keep the kneecap centred. MPFL reconstruction aims to restore this stabilising function.

Who may benefit from patella stabilisation?

Surgery may be considered for recurrent patella dislocations, persistent apprehension, high-risk anatomy, loose bodies, cartilage injury or failure of non-operative rehabilitation. Imaging helps assess trochlear shape, patella height, alignment and cartilage damage.

What does the procedure involve?

MPFL reconstruction uses a tendon graft to recreate the stabilising ligament on the inner side of the knee. Additional procedures may be required if bony alignment or tracking factors are significant.

Recovery after MPFL reconstruction

Recovery focuses on swelling control, restoring motion, quadriceps activation and then progressive strengthening. Return to sport requires strength, control and confidence.

Rehabilitation protocol

You will receive an MPFL reconstruction protocol outlining brace use, motion progression, strengthening and return to sport.

FAQs

Does every kneecap dislocation need surgery?

No. First-time dislocations are often treated non-operatively unless there are high-risk features or loose fragments.

What does MPFL reconstruction do?

It recreates the ligament that helps stop the kneecap dislocating laterally.

Will I need a brace?

A brace may be used early depending on the procedure and surgeon preference.

When can I return to sport?

Return depends on strength, control, confidence and sport demands.

Can instability recur?

Recurrence is possible, particularly if underlying bony factors are significant.
