TFCC Repair and Ulna Shortening

Overview

The TFCC is a stabilising structure on the little-finger side of the wrist. It helps support the distal radioulnar joint and cushions load across the ulnar side of the wrist. TFCC injury can cause pain with twisting, gripping, push-ups, racquet sports or weight-bearing through the hand.

Ulna shortening may be considered when ulnar-sided wrist overload contributes to symptoms, particularly when the ulna is relatively long compared with the radius.

Who may benefit from surgery?

Surgery may be considered for persistent ulnar-sided wrist pain, confirmed TFCC tear, distal radioulnar joint instability or ulnar impaction that has not improved with splinting, injections or therapy.

What does the procedure involve?

Arthroscopy is used to assess the TFCC. Stable central tears may be debrided. Peripheral tears with healing potential may be repaired. Ulna shortening osteotomy removes a small section of ulna and fixes the bone with a plate to reduce overload on the ulnar side of the wrist.

Recovery after TFCC repair or ulna shortening

TFCC repair requires protection while soft tissues heal. Ulna shortening also requires bone healing. Return to loading is gradual and guided by comfort, imaging and strength recovery.
